Information for Lawyers Referring Patients to the NMCI Medical Clinic
When referring new patients to NMCI, please include the following documents in your email or fax:
- 4600 letter
- Patient demographics
- Medical records
- Status of the claim
To facilitate prompt scheduling for your clients, it is imperative you fax the documents listed above to 408-498-4552 or email them to email@example.com. For additional information, you can also contact our New Patient Department at 408-217-0355 Ext. 307 or Ext. 327. Any questions regarding authorizations, please contact our Authorization Department at Ext. 309.
Workers Compensation Experts
We provide medical legal services such as permanent and stationary reports, QME, and AME evaluations. Additionally, our providers can perform Functional Capacity Evaluations (FCE’s) to assess our patients work limitations so they can safely and independently perform their job duties.
We offer a functional restoration program (FRP) designed to help patients cope with chronic pain and whom have had inadequate outcomes from prior treatment and medications. Through this program, you will learn to reduce the negative impacts of chronic pain including depression, anxiety, and social isolation.
